You will need a state issued ID card or Drivers License, your parent will need their ID card / Drivers License, and a copy of the birth certificate may be required to prove guardianship.

If you don't have a License / ID card, you will need something with your Photo and Name printed on it. Such as a library card, club membership card or yearbook.
If you and your parent/guardian have different last names (happens ALL the time), you will have to bring your birth certificate and the above mentioned ID(s)
The main thing is this;
You have to prove to us (and the Health Department) that you are who you say you are and that your parent or guardian are in fact your parent or guardian.
